I hereby acknowledge and agree to the terms of this Liability Waiver for participating in St. Katherine Greek Orthodox Church's Youth Ministry activities and events during the 2024-25 school year.
1. Activities and Events: I understand that my child may participate in various activities and events organized by St. Katherine Greek Orthodox Church, Inc. (Church) during the 2024-25 school year, both on church grounds and at other venues. These activities (Youth Ministry Activities) may include, but are not limited to, Bible studies, worship services, retreats, community service projects, and recreational activities.
2. Transportation: I grant permission for my child to travel in vehicles driven by authorized Church youth advisors for Youth Ministry Activities, understanding that all drivers will adhere to traffic laws and exercise responsible driving practices.
3. Assumption of Risk: I acknowledge that there are inherent risks associated with participating in Youth Ministry Activities, both on and off Church premises. I understand that despite precautions taken by the Church, accidents, injuries, or medical emergencies may occur.
4. Release of Liability: In consideration of my child being allowed to participate in Youth Ministry Activities, I, on behalf of myself, my child, our respective heirs, personal representatives, and assigns, do hereby release, discharge and agree to jointly and severally indemnify and hold harmless the Church, its officers, directors, employees, staff, volunteers, and agents from any and all claims, liabilities, injuries, damages, or losses, together with reasonable attorneys’ fees and court costs, arising out of or in connection with my child's participation in these Youth Ministry Activities, including, but not limited to, any travel to and from these Activities.
5. Medical Treatment Authorization: In the event of an emergency or medical situation during Youth Ministry Activities, I authorize the Church's youth advisors or their designees to seek medical attention for my child and grant permission for any necessary medical treatment or procedures to be administered. The Parents/Guardian further agree to indemnify and reimburse the foregoing parties for any and all costs or expenses associated with any and all medical treatment or procedures administered.
6. Insurance Information: I understand the importance of providing accurate insurance information for my child. In case of an emergency.
By signing this Liability Waiver, I acknowledge that I have carefully read and understood its contents. I agree to abide by all rules and regulations set forth by St. Katherine Greek Orthodox Church, Inc. during the 2024-25 school year for Youth Ministry Activities.
